Advancing precision in thyroid diagnostics

How modern imaging, structured reporting and emerging technologies are improving thyroid assessment and patient care

Thyroid nodules are very common, with around 25–30% of adults found to have a nodule or cyst on imaging. The vast majority are benign and remain clinically silent throughout life. Even when cancer is present, most thyroid malignancies are slow-growing and respond well to treatment.

However, according to Dr Astrid Schweitzer, Head and Neck Imaging Lead at Unilabs’ Telemedicine Clinic (TMC), early recognition is crucial for the smaller group of clinically aggressive thyroid cancers.

“When more aggressive disease is identified before it extends to surrounding structures, outcomes can be significantly improved. Timely, carefully selected treatment, whether surgery, radioiodine in appropriate cases or targeted molecular therapies, can make a real difference.”

The strength of modern imaging

Greater accessibility and accuracy in thyroid assessment

Radiology has significantly improved the accessibility and accuracy of thyroid assessment. High-resolution ultrasound remains the cornerstone of thyroid imaging and, when used selectively, computed tomography (CT) and magnetic resonance imaging (MRI) provide valuable complementary information in complex or advanced cases.

Together, these tools support clinicians in detecting even small abnormalities with greater confidence.

At the same time, Dr Schweitzer notes that increased sensitivity also has potential downsides.

“The ability to see very small thyroid changes reliably is a strength, but it also increases the risk of detecting abnormalities that are clinically irrelevant. Over-detection and over-reporting can have real consequences for our patients and the health systems.”

Even suggesting a possible malignancy, particularly a low-risk tumour, can understandably increase anxiety or lead to further investigations or procedures that may not always improve outcomes.

“The challenge is to focus on findings that are truly meaningful, so patients receive appropriate follow-up when needed, and reassurance when it is not.”

A more structured, evidence-based approach

Using standardised systems to support clearer decisions

To address this challenge, thyroid imaging has evolved towards standardised, evidence-based reporting.

Systems such as the Thyroid Imaging Reporting and Data System (TI-RADS) help radiologists and clinicians assess risk more consistently and make clearer decisions regarding biopsy and follow-up.

“These frameworks have been extremely valuable,” Dr Schweitzer explains. “They help distinguish nodules that genuinely warrant intervention from those that can safely be left alone, bringing more clarity to both clinicians and patients.”

Enhancing insight with advanced technologies

Molecular diagnostics and AI can support more personalised risk assessment

While ultrasound remains central, additional diagnostic tools are increasingly refining thyroid care.

Dr Schweitzer explains that molecular testing can provide additional risk information when biopsy results are indeterminate, while AI-supported analysis may help reduce variability in ultrasound interpretation.

“We are seeing cytology and histology increasingly complemented by molecular diagnostics. Together, these approaches improve risk stratification and support more confident decisions — including when surgery is appropriate, and how extensive it should be.”

Technology does not replace clinical expertise.

“These tools support radiologists and clinicians by adding structured, data-driven insight to complex cases. The goal is better-informed, patient-specific decisions, not automation for its own sake.”

Looking ahead, AI-driven decision support and personalised risk assessment are expected to continue refining thyroid diagnostics, improving efficiency while maintaining high standards of care.

Clear reporting and multidisciplinary collaboration support better patient care

Clear, structured and evidence-based reporting plays a central role in ensuring that technological advances translate into real clinical benefit.

Well-designed reports support sound clinical decisions and help avoid unnecessary follow-up. They ensure that significant disease is identified promptly, while patients with low-risk findings receive appropriate reassurance.

Recent guidance, including the European Society of Radiology (ESR) Essentials recommendations on thyroid imaging, has further supported this shift.

“At TMC, these recommendations have helped shape a more consistent and clinically focused reporting approach,” Dr Schweitzer says. “They reflect the ability to refine our frameworks continuously, so they better serve both clinicians and patients.”

Equally important is close collaboration across specialties.

“When imaging findings, laboratory results and clinical context are considered together, decision-making becomes more holistic and better aligned with patient needs. That integration is essential for continuity of care and appropriate management.”
