I seguenti termini e condizioni regolano la relazione tra Unilabs Laboratoire d'analyses médicales SA, Chemin des Perrières 2, 1296 Coppet e il cliente del servizio Unilabs GO accessibile su https://go.unilabs.ch.
1 Prezzo e pagamento
Tutti i prezzi includono l'imposta sul valore aggiunto prevista dalla legge.
Il pagamento viene effettuato tramite carta di credito (VISA, Mastercard, American Express, PostFinance) o TWINT. L'importo sarà addebitato immediatamente sul mezzo di pagamento con la conferma della prenotazione dell'appuntamento.
2 Sconti
Se viene usato un codice di sconto al momento della prenotazione verrà addebitato sul mezzo di pagamento utilizzato solo l'importo ridotto.
Per ogni prenotazione può essere riscosso solo un codice di sconto. Il pagamento in contanti del valore dello sconto non è possibile.
Un codice di sconto deve essere inserito obbligatoriamente prima di completare la prenotazione. Un inserimento successivo non è possibile.
Il giustificativo di legittimazione per il codice di sconto deve essere presentato direttamente sul posto al centro di test. Se non è possibile presentare un valido giustificativo di legittimazione, non verrà effettuato alcun test e l'importo addebitato sarà rimborsato sul mezzo di pagamento utilizzato al momento della prenotazione.
3 Nessuna assunzione di costi da parte dell'assicurazione malattie
I costi del test PCR senza prescrizione medica sono interamente a carico del cliente. Un rimborso dei costi del test da parte dell'assicurazione malattie è – anche successivamente – escluso.
4 Cancellazione della prenotazione
La cancellazione dell'appuntamento è possibile gratuitamente fino a quattro ore prima dell'appuntamento. L'importo addebitato sarà rimborsato sul mezzo di pagamento utilizzato al momento della prenotazione.
Se la prenotazione viene cancellata meno di quattro ore prima dell'appuntamento, i costi saranno addebitati per intero. L'importo addebitato non sarà rimborsato.
5 Esattezza dei dati
Il cliente è responsabile dell'esattezza dei dati personali forniti. Unilabs non è tenuta a verificare l'esattezza di questi dati.
Se il cliente fornisce dati errati (nome errato o incompleto, data di nascita errata, numero di cellulare errato, scelta errata del centro di test, ecc.) potrebbe non essere possibile effettuare un test. I costi del test in questi casi non saranno rimborsati e si esclude qualsiasi responsabilità da parte di Unilabs.
6 Identificazione sul posto
Il cliente deve identificarsi sul posto al centro di test con un documento d'identità ufficiale con foto (p. es. carta d'identità o passaporto).
Se non è in grado di presentare un documento d'identità valido, il test non può essere effettuato. I costi del test in questi casi saranno rimborsati per intero sul mezzo di pagamento utilizzato al momento della prenotazione e si esclude qualsiasi responsabilità da parte di Unilabs.
7 Protezione di dati e condizioni generali
Con il completamento della prenotazione il cliente accetta la Privacy Policy e la Legal Notice di Unilabs.
8 Perdita di campioni
Se un campione viene perso, i costi dell'analisi saranno rimborsati sul mezzo di pagamento utilizzato al momento della prenotazione e si esclude qualsiasi responsabilità da parte di Unilabs.
In rari casi potrebbe essere necessario ripetere il test in laboratorio per ottenere un risultato valido. Questo non comporterà alcun costo aggiuntivo per il cliente. Si esclude qualsiasi responsabilità da parte di Unilabs per danni causati dalla ripetizione del test e dalla relativa trasmissione ritardata del risultato del test.
9 Risultato del test
Si esclude qualsiasi responsabilità da parte di Unilabs per danni causati da una trasmissione ritardata del risultato del test.
Il risultato del test viene trasmesso al cliente per via elettronica. La trasmissione scritta del risultato del test in forma cartacea (rapporto cartaceo) è esclusa.
Il cliente riceve il risultato del test tramite un link inviato per SMS al numero di cellulare fornito.
Se non è possibile inviare un SMS (problemi di connessione, ecc.), il risultato viene inviato all'indirizzo e-mail fornito.

These terms and conditions govern the relationship between Unilabs Laboratoire d'analyses médicales SA, Chemin des Perrières 2, 1296 Coppet and the customer of the Unilabs GO service accessible at https://go.unilabs.ch.
1 Prices and payment
All prices are inclusive of the statutory value added tax.
Payment is made by credit card (VISA, Mastercard, American Express, PostFinance) or TWINT. The amount will be charged to the means of payment immediately upon confirmation of the appointment booking.
2 Discounts
When using a discount code at the time of booking, only the reduced amount will be charged to the means of payment used.
Only one discount code is redeemable per appointment. A cash payment of the discount value is not possible.
A discount code must be entered before the booking is completed. A subsequent entry is not possible.
Proof of eligibility for the discount code must be presented at the on-site test center. If no valid proof of eligibility can be presented, no test will be performed, and the amount charged will be refunded to the means of payment used at the time of booking.
3 No coverage by health insurance
The costs for a PCR test without prescription are to be borne in full by the customer. Reimbursement of the test costs by the health insurance is excluded - even retrospectively.
4 Cancellation of the booking
Cancellation of the appointment is possible free of charge up to four hours before the appointment. The amount charged will be refunded to the means of payment used for the booking.
If the appointment is cancelled less than four hours before the appointment, the costs will be charged in full. The amount charged will not be refunded.
5 Accuracy of the data
You are responsible for ensuring that the personal details entered are correct. Unilabs is not obliged to check the accuracy of this information.
If the customer enters incorrect data (incorrect or incomplete name, incorrect date of birth, incorrect entry of mobile phone number, incorrect choice of test center, etc.), it may not be possible to carry out a test. The costs for the test will not be reimbursed in these cases and any liability by Unilabs is excluded.
6 Identification on site
You must identify yourself on site at the test center by means of an official form of identification with a photograph (e.g. identity card or passport).
If you are unable to provide valid proof of identity, a test cannot be carried out. In such cases, the cost of the test will be refunded and credited to the means of payment used for the booking, and any liability on the part of Unilabs is excluded.
7 Data protection and legal terms
By completing the booking you agree to the Privacy Policy and the Legal Notice of Unilabs.
8 Loss of sample
If a sample is lost, the cost of the analysis will be refunded and credited to the means of payment used for the booking, and any liability on the part of Unilabs is excluded.
In rare cases, the test may need to be repeated in the laboratory to obtain a valid test result. This does not result in any additional costs for you. The liability of Unilabs for damages caused by repeating the test and the associated delayed transmission of the test result is excluded.
9 Test result
The liability of Unilabs for damages caused by a delayed transmission of the test result is excluded.
The test result will be sent to you electronically. The written transmission of the test result in paper form (paper report) is excluded.
You will receive the test result with a link via SMS to the mobile phone number you provided.
If no SMS can be sent to you (connection problems, etc.), the result will be sent to the e-mail address you specified.
